Health Innovation Fund - Stage 2 - WA

This Schedule will support the delivery of the following projects: Community Health in a Virtual Environment (Co-HIVE) - Mental Health Model; Evaluation and Small Capacity Building Grants for Community­based Aged Care Transition Care Program (TCP) Services for Aboriginal Patients; Pilot of Transition Care Arrangements for Patients with a Disability; Digital Readiness; Comprehensive Healthcare for People Experiencing Homelessness; and WA Cancer Registry.

Health Innovation Fund - Stage 2 - NSW

This Schedule will support the delivery of the following projects: Harnessing data to deliver better health; the Rural and regional clinical data sharing system; the Genomics Integrated and Automated ICT Workflows; and a Maternal Safety Intelligence Register.

Health Innovation Fund - Stage 2 - ACT

This Schedule will support the delivery of an integrated model of care which aims to maintain wellbeing for high-risk populations, and a First 1,000 Days Action Plan (Best Start) to address domains identified by the Best Start: First 1000 days initiative as being critical to the success of children in the first 1000 days of their life.

Health Innovation Fund - Stage 2 - SA

This Schedule will support the delivery of the Priority Care Centre (PCC) program, phase two. This project expands on the Primary Care Centre (PCC) program which provides community care to patients who would otherwise present to an emergency department.

Prevention of Avoidable Hospital Admissions, Obesity and Chronic Disease

This Schedule will support the delivery of:
• Improving outcomes for people with cardiovascular disease,
comprising of:
• An Enduring Data Linkage Strategy ($3.495 million) which
routinely identifies patients with cardiovascular disease at risk
for hospitalisation.
• Three improvement initiatives ($8.505 million) including an
Improvement Collaborative focused on reducing readmission
for heart failure patients, the ‘Health Helper’ program and the
Cardiovascular Ambassadors nurse program.
• Development of integrated Public Health Syndromic Surveillance
system ($3.0 million) to rapidly identify any unusual patterns of
illness that may indicate a risk to public health, across emergency
departments and other health services, so that an early and
appropriate public health response can be stood up.
• An obesity and chronic disease prevention initiative ($8.0 million)
for children, targeted to high priority areas and focused on boosting
healthy eating among children and their families.

Community Health and Hospitals Program and other Initiatives for the NT

This Schedule will support the delivery of:
* Design and construction of a 12 single person accommodation units at the Tennant Creek Hospital.
* Construction of a new purpose built facility that will enhance and expand the provision of ambulatory care services at the Alice Springs Hospital.
* Mental Health inpatient Facility.
