Palliative Care Services Navigation Pilot

This Schedule will support the delivery of a pilot to test a palliative care service navigation model. The Palliative Care Services Navigation Pilot will develop a better supported and more comprehensive approach into end-of-life care pathways, for the benefit of the patient, carers and the health system, through increasing access to and uptake of palliative care via a multi-channel approach.
